Fraud Alert – Bogus Unclaimed Rebate Fraud Form
Members should be aware that there appears to be a fraudulent unclaimed rebate form using the name of Office Depot being sent by mail. The form requests the FEIN or personal social security numbers of recipients. The form alleges there is an unpaid rebate available to individuals who complete the information. The form uses the Office Depot name and looks suspiciously like a copy of their logo. The form directs rebate recipients to send the form to an address in Boca Raton, Florida but no phone number is given. When we called Office Depot, they disavowed any knowledge of the unclaimed rebate form.

Please respond and gives us your feedback on this year’s program and event and tell us what you would like to see next year. We mail the surveys to every attendee. That’s the only way to get them directly to each person who signs up for the program. We do not have the e-mail addresses for all those who attended, so we mail the surveys -- once. If you haven’t received a survey by the end of week, give us a call on Monday. Set aside the dates of December 7 – 9 in Salt Lake City and make the most of the event by bringing staff and getting them charged up by the event.
